چکیده

Abstract Aim The ‘Basket-in-catheter’ (BIC) technique facilitates laparoscopic transcystic ductal exploration (LTCE) and increases its success rate, being easier safer than inserting the basket alone. This study evaluates benefits in confirmed suspected stones. Method Prospective preoperative, operative postoperative data on consecutive single session explorations was collected over 28 years analysed. BIC became our default for approach to or bile duct Results 741 of 1225 (60.5%) attempted LTCE were performed using retrieval baskets without dilating cystic (CD). used 646 (87.2%). Of 386 (52.1%) patients undergoing successful stone 62.7% had clinical radiological risk factors stones 92.0% positive intraoperative cholangiography. 355 (47.9%) preoperative CBD equivocal cholangiography 25%. Basket trawling negative repeat resolution abnormalities. Choledochoscopy utilised 484/1225 (39.5%), either primarily when blind failed extract Retained occurred 7 patients, six requiring ERCP. Bile leakage 6 patients. There two open conversions, no biliary injuries mortality. Post-operative pancreatitis recurrent 8 Conclusions achieves CD dilatation 40%, reducing need choledochoscopy choledochotomy. It safe speedy are due helps surgeons acquire consolidate skills.

Transcystic Approach to Laparoscopic Common Bile Duct Exploration

BACKGROUND AND OBJECTIVES One-stage laparoscopic management for common bile duct stones in patients with gallbladder stones has gained wide acceptance. We developed a novel technique using a transcystic approach for common bile duct exploration as an alternative to the existing procedures. METHODS From April 2010 to June 2012, 9 consecutive patients diagnosed with cholelithiasis and common bi...

Laparoscopic Transcystic Common Bile Duct Exploration: Advantages over Laparoscopic Choledochotomy

PURPOSE The ideal treatment for choledocholithiasis should be simple, readily available, reliable, minimally invasive and cost-effective for patients. We performed this study to compare the benefits and drawbacks of different laparoscopic approaches (transcystic and choledochotomy) for removal of common bile duct stones. METHODS A systematic search was implemented for relevant literature usin...

In Canada, the adjusted annual rate of elective cholecystectomy was 260.8 per 100 000 population during the period 1992–2000. Gallstone disease is the second most com...

A forward-pulling technique for complicated Trapezoid basket impaction during retrieval of common bile duct stones.

Although basket impaction rarely occurs during retrieval of common bile duct stones, it is a potentially serious event [1]. The Trapezoid RX wire-guided retrieval basket (Boston Scientific Corp., Marlborough, Massachusetts, USA) has an emergency release feature, whereby the tip is designed to disengage, thereby facilitating basket removal and reducing the risk of impaction [2]. However, we enco...

Linear endoscopic ultrasound for clinically suspected bile duct stones.

BACKGROUND Radial endoscopic ultrasound (EUS) is a less invasive imaging modality used to evaluate common bile duct stones (CBDS) and is as accurate as endoscopic retrograde cholangiography (ERCP). Patients classified as high risk for suspected CBDS were found to have stones in only two-thirds of cases, and an argument could be made for performing EUS in these high-risk patients. There are very...
